Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A renewal inspection was initiated on 12/10/2020 and concluded on 12/15/2020. The director was contacted by telephone to initiate the inspection. There were 31 children present and 5 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility.
The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.
Based on review of two children's records, the center failed to ensure that all required items were obtained.
Evidence: 1. Child # 2 started the program 12/14/2020 and was missing chronic physical problems and physician and contact phone number.
Plan of Correction:
The director will make sure that all forms are completed and will monitor them as they come in.

Description:
Based on review of two staff's records, the center failed to ensure that all new hires had a CPS background check within the 30 days of hire.
Evidence 1. Staff # 2 was hired on 10/05/2020 and on the day of the inspection a CPS has not been returned nor has documentation of contact with Office of Background Information.
Plan of Correction:
The director stated that she has attempted to contact OBI on several occasions with no luck in reaching anyone. The director did not document any contact with OBI. She will resubmit the CPS check today and will send a copy to the Licensing Office.
