Comments:
An unannounced renewal inspection was conducted today from 9:00am-11:30pm. There were 85 children (infants-5yrs.) supervised by 14 direct care staff. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, reading books, doing an art project and circle time. Diapering and hand-washing procedures were also observed. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Based on review of children's records, an annual parental review of emergency contacts is not being requested. Evidence: Children's records do not have documentation of annual parental review of emergency contacts.

We are going to email out the emergency contact form to have parents complete so we have a new one for each family.

Description:
Based on observation, the diapering surface was not cleaned and sanitized according to licensing standards after a diaper change. Evidence: In the Infant room, the diapering surface was sprayed with soap and water to clean, but was not sprayed with a solution of bleach and water and allowed to air dry after a diaper change.

I will review with all staff again the correct procedures.

Description:
Based on observation, the posted menu did not reflect what was provided to children. Evidence: Cereal was listed as the AM snack for today, biscuits was actually served.
Plan of Correction:
We will make sure to update the posted menu when we make changes.
