This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.
A subsidy monitoring inspection was initiated on January 29, 2021 and concluded on February 8, 2021. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the non-compliance with subsidy regulations, and violations were issued.
Based on interviews and documentation review, the center did not notify the department within two business days of a serious injury to a child - while under the center's supervision - that required outside medical attention.
Evidence: A child sustained an injury to their leg while in the center's care on January 21, 2021. The parents took the child to the Doctor for examination. On January 26, 2021, the center completed an Injury Requiring Outside Medical Treatment (IROMT) online report.
