Technical Assistance:
The provider was informed that background checks are due to expire for the provider and the assistant in June 2023. The provider was also informed that the tuberculosis screening for the assistant will expire in June 2023.
Comments:
An unannounced renewal inspection was conducted on June 9 2023 between 11:45am and 12:45pm. There were 7 children (21 points) in the direct care of 1 caregiver. A second caregiver arrived at approximately 12:20pm. Children were observed napping throughout the inspection. A review of caregiver and child records was conducted. The emergency kits, attendance record, evacuation drills, flashlight, weather radio, and they physical space of the family day home was observed and inspected. Based on record review, the provider did not obtain, for each caregiver, a current Report of Tuberculosis Screening Form every two years from the date of the first screening.
Evidence: Caregiver #1 record contained documentation of a Tuberculosis screening dated November 19 2020.

Description:
Based on direct observation, the provider did not ensure areas and furnishings of the family day home, inside and outside, were maintained in a safe and operable condition.
Evidence: Outside in the play area, A wooden playhouse was not observed to be sturdy. With minimal force the structure would move. The play structure was also observed to have jagged wooden edges, and loose interior boards.

Based on direct observation, the provider did not ensure that a caregiver did not exceed 16 points by using the family day home point system.
Evidence: During the inspection Caregiver #2 was alone with 7 children (21 points) for approximately 35 minutes. Caregiver #1 arrived at 12:20 pm. Children were observed napping during this time frame.
