Comments:
An unannounced monitoring inspection was conducted on 7/28/22 from 9:55am - 11:00am. During the inspection there were 20 children ages five years old through eight years old in care with two staff. Children were observed participating in various activities in the classroom. Records were reviewed for three staff during the inspection. There is no medication at the facility. Emergency procedures and emergency supplies were reviewed during the inspection. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.

Violations:
Standard #: 22.1-289.031-A-8
Description: Base don a review of three staff records, it was determined that the provider did not ensure that there is a person trained and certified in first aid and cardiopulmonary resuscitation (CPR) will be present at the child day center whenever children are present or at any other location in which children attending the child day center are present.

Evidence:
1. None of the three records reviewed contained documentation of a current certification in first aid and cardiopulmonary resuscitation (CPR).
2. Staff #1 confirmed that there was no staff present during the inspection with a current certification in first aid and cardiopulmonary resuscitation (CPR).

Plan of Correction: The provider responded: We will ensure all staff complete a certification in first and and CPR.

Standard #: 22.1-289.031-B-2
Description: Based on a review of three staff records, it was determined that the facility did not ensure that an employee is allowed to begin employment without a completed national criminal history record check (finger printing).

Evidence:
1. The record for staff #2 did not contain documentation of a completed national criminal history record check (finger printing).
2. The record for staff #3, working during the inspection did not contain documentation of a completed national criminal history record check (finger printing).
3. Staff #1 (Program Director) reviewed the record for the staff #2 and staff #3, and confirmed that she had not received the results of the national criminal record check (finger printing).

Plan of Correction: The provider responded: Staff #3 will complete a criminal record check. All new staff will complete a criminal record prior to beginning employment.

Standard #: 8VAC20-770-40-D-2
Description: Based on a review of three staff records, it was determined that the facility ensure that all staff have a completed search of the central registry finding within 30 days of employment.

Evidence:
1. The record for staff #3, working during the inspection, did not contain documentation of a completed search of the central registry finding.
2. Staff #1 (program Director) confirmed that staff #1 did not have a completed search of the central registry finding.
2. Staff #6 (Program Director) reviewed the record for the staff #1, and confirmed that the search of the central registry finding has not been received.

Plan of Correction: The provider responded: Staff #3 will complete a search of the central registry check. All new staff will complete a search of the central registry within 30 days of employment.
