Conducted an unannounced inspection at 10:30am. Observed 56 children + 18 direct-care staff. Ratios and supervision were in compliance. Children were observed in a variety of activities, to include: writing the letter ?e + E?, story time, washing hands, eating snack, and going outside for active play in a nearby park. As part of the language emersion program interactions between children and staff were in Spanish, French, and Mandarin, as applicable to a given class. Staff were actively engaged with the children and leading activities. Classrooms were found to be clean and well supplied with equipment for the children. Based on records reviewed, it was determined that 1 of 3 staff records did not include documentation of a required background check.
Evidence:
1. Staff #6 ? There was no documentation of a completed central registry check (CPS). 2. The 1st date of employment was 7/18/22. 3. A mailing receipt dated 10/17/22 was provided to the inspector as proof of the CPS check being mailed to the State for processing. 4. The CPS check was not submitted within the 1st 30-days of employment.
Plan of Correction:
We had a meeting with HR team. The HR team could not use the website because it was down. They will follow up with you regarding that. They will make sure to do the background check within the first 30 days of employment.

Description:
Based on observations made, it was determined that an electrical outlet in one classroom did not have protective safety covers. Evidence: A low-level outlet, under a window, in a 3-4 year old classroom did not have a required safety cover.
Plan of Correction:
We will keep communicating with all the staff verbal and written to make sure that all the outlets in the classroom are always covered and being checked in the daily bases. We have contacted our cleaning company to make sure to put back the cover once using an outlet in school.
