Technical Assistance:
Standard 22VAC40-111-(7)-320-G related to documentation for wood burning stoves and fireplaces was discussed.
Comments:
An unannounced Monitoring Inspection was conducted on September 10, 2019 from approximately 10:30 am until 1:30 pm. Upon entrance there were 8 children, totaling 21 points and 2 caregivers. The record review sample consisted of 5 children's records, 1 provider record, 1 caregiver record and 2 household members records. Children were observed freely playing, eating lunch and during their rest period. The results of the inspection were reviewed and verified with the provider during the exit interview. Based on documentation review and provider interviews, the provider did not ensure that all required telephone numbers were posted.
Evidence: There was not a telephone number posted for the designated responsible person for emergency backup care.

Based on a review of 5 children's records and provider interviews, the provider did not ensure there was an up-to-date record for each child.
Evidence: The following records were determined incomplete 1) The record for child #2 did not include the house numbers or apartment numbers for the 2 emergency contacts listed. 2) The record for child #4 did not include the policy number for health insurance, the house number or apartment number and the street name, state and zip code for 1 emergency contact listed, the last names of the complete names of the 2 emergency contact listed (missing last names) and information regarding attending another program/school. 3) The record for child #5 did not include the policy number for health insurance.

Provider will have parents update records today.
