Comments:
An unannounced monitoring inspection was conducted from 9:15 - 10:45 am with the provider. There were nine children in care, ranging from 16 months to five years of age, for a total of 19 points. The children were observed having free play in various centers including the play kitchen, reading, puzzles, dinosaurs and building and going outside to play. Four children's records and four caregiver/household member records were reviewed. The emergency drill log was reviewed and in compliance. Required information is posted. The provider is not Medication Administration Training (MAT) certified and cannot administer medications to children in care. Based on a review of four provider/household member records, and an interview with the provider, it was determined that repeat tuberculosis (TB) test/screenings were not completed every two years. Evidence: The TB test for provider #2 is dated: 8/9/17 and for household member #1 the TB test is dated: 1/8/17.
Plan of Correction:
Provider and household member will obtain the updated TB test/screening.

Description:
Based on observation, it was determined that one caregiver was left alone with 9 children for a total of 19 points. Evidence: Upon entering the family day home the Licensing Inspector observed that the provider was alone with 9 children, ages 3 years, 18 months, 4 years, 16 months, 4 years, 3 years, 5 years, 2 years and 2 years for a total of 19 points. Approximately 30 minutes after arriving, the assistant arrived to assist in supervising the children.
Plan of Correction:
Will call a substitute if the assistant is unable to arrive on time.
