Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 7/28/2020 and concluded on 7/28/2020. The provider was contacted by telephone to initiate the inspection. There were five children present with the provider supervising. The inspector emailed the provider a list of items required to complete the inspection. The Inspector reviewed four children's records and four provider/household member records to ensure documentation was complete.
The information gathered during the inspection determined non-compliance with standards that are documented on the violation notice issued to the facility.

The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs. If you have further questions about this inspection please contact Donna Liberman at 540-359-5244 or Donna.Liberman@dss.virginia.gov.

Violations:
Standard #: 22VAC40-111-60-B
Description: Based on review of four children's records, it was determined that not all records contained all required information. Evidence: the records for child #3 and child #4 did not have the addresses for two emergency contacts or the medical insurance name and policy number.

Plan of Correction: Parent was contacted during inspection for missing information.

Standard #: 22VAC40-111-180-A
Description: Based on review of four provider/household member records, not all caregivers/household members had a repeat tuberculosis (TB) test/screening at least every two years from the date of the initial screening or testing. Evidence: the TB test/screening for household member #3 is dated 1/8/2017.

Plan of Correction: Will obtain updated test/screening when feasible due to COVID-19.
