An unannounced joint investigation was conducted with a representative from the local agency and the provider. The investigation was initiated on August 7, 2023 at 2:00pm and concluded on August 22, 2023. Interviews were conducted and documentation was reviewed. The allegations of behavioral guidance and forbidden action were unable to be substantiated due to insufficient corroborating evidence.
