Comments:
An unannounced complaint investigation was conducted on 3/10/2020 from 2:00pm to 4:00pm, in response to a complaint that was received by the Fairfax licensing office on 2/28/2020, regarding programs and supervision. Based upon interviews, records reviewed and observations made, the preponderance of evidence gathered during this investigation does not support the allegation. The complaint is determined to be not valid; however, there is a non-related violation identified on the Violation Notice. Based on record review, children's records did not all contain required information.
Evidence: Child#1's record is missing 2 out of 2 required emergency contacts and a written authorization for emergency medical care, should an emergency occur and the parent cannot be located.
Plan of Correction:
Parents have not returned since the pandemic started. Date to be corrected: When parents return back to school.
