Comments:
An unannounced complaint inspection was conducted on site on 7/25/2022. The inspectors arrived at 12:00 PM and departed at 1:45 P.M. Follow up interviews were completed on 7/27/2022. A complaint was received in the licensing office on 7/25/2022. The preponderance of evidence of no lock down plan was supported and complaint is valid.

Based on interviews, the center failed to ensure that the emergency plan included procedures for a lock down.
Evidence: 1.Staff # 1 stated that the lock down plan could not be located on the day of the complaint inspection.
