Comments:
An unannounced self-report inspection was conducted on site on 8/31/2022. The self-report was received on August 26, 2022. The preponderance of evidence of sight and sound supervision was supported.

Based on review of the center?s policies and procedures the center failed to ensure that the center maintains compliance with the center?s own policies and procedures that are required by the standards.
Evidence: 1.Procedure for identifying where attending children are at all times was not followed by staff on the day of the incident.
Plan of Correction:
-We immediately made the staff lanyards with the class roster on it. Teachers will mark off students that are absent and keep an accurate head count on them. The exception is the infant teachers, unless they are outside. -Teachers also keep an accurate head count on their glass door. -We immediately had a staff training Wednesday, August 31 at 7:30 where we reviewed Ratios and Playground Safety Policy & Procedures.

Description:
Based on interviews and video footage, the center failed to ensure that all children were within sight and sound supervision of a staff member.
Evidence: On 8/26/2022, Child # 1, age 3, was left out of sight and sound supervision for approximately 56 minutes. There were 13 children in care and staff did not count the last child that arrived into the program at approximately 9:16 AM. At approximately 11:20 AM, 12 children lined up to exit the building and Child #1 was left in the classroom asleep in the reading chair. Staff #1 and Staff #2 stated that they were using the initial head count of 12 children and not 13 when they lined up to exit. At approximately 12:08 PM, Child # 1 awakes and wanders around the classroom. At approximately 12:16 PM, the class returned from the playground and Child # 1 was discovered.
