Comments:
An unannounced monitoring inspection was conducted on 04/17/2024 from 2:00 p.m. to 2:40 p.m. regarding a self report that was received on 04/05/2024. The inspector reviewed compliance in the areas of staffing and supervision.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on interview, the caregiver failed to actively supervise each child during outdoor play. Evidence: On 04/05/2024, the caregiver (provider) went inside with one child, leaving eight children (ages two to ten years old) outside. While inside, the caregiver saw Child #1, three years old, walk down the driveway towards West Main Street (2 lanes, 25 miles per hour). Before the provider was able to catch Child #1, a passerby stopped and blocked traffic as Child #1 had made it to West Main Street.
Plan of Correction:
We have changed the way we go outside. Have placed a lock on gate. Making sure if I have to com in to bring everyone under age of 7 inside.
