Comments:
An unannounced mandated inspection was conducted on September 19, 2019 from 9:40 A.M.-12:30 P.M. There were 7 children in the care of two staff members. Five children records, three staff records, one volunteer record, evacuation drills, emergency plan, first aid kit and emergency supplies, health inspection,fire inspection, injury prevention plan, playground safety plan, outdoor play area, and required posted information were reviewed. Children were observed during during handwashing, interest center activities that included art activities, blocks, dramatic play, small group activities that included an activity with fine motor skills, snack time, outdoor play time, music and movement, and departure. The exceptions to compliance are noted on the violation notice.

Based on observation, the center failed to post shelter-in-place procedures/maps.
Evidence: 1. The emergency maps located in the classrooms did not indicate the shelter-in-place location. 2. Staff verified that the map did not indicate the shelter-in-place location.
Plan of Correction:
Administration immediately placed the shelter-in-place location on all emergency maps in the facility.
