Comments:
An unannounced monitoring inspection was conducted 8/31/2022 from 10:45 am-12:15pm. There were 9 children (23 points), between the ages of 11 months and 4 years old, with 1 provider for a total of 23 points exceeding the supervision and capacity guidelines for approximately 15 minutes. The home, 2 staff records, 5 children?s records, 2 household member?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed engaging freely. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. Based on record review and interview, the provider did not review the emergency plan at least annually and update the plan as needed.
Evidence: The emergency plan was last reviewed on 5/19/2020.

Based on observation and interview, a landline telephone was not available during the family day home?s hours of operation.
Evidence: The provider stated she did not have a landline phone in the home.

Based on observation, the provider did not ensure that the point system of 16 points per caregiver was followed.
Evidence: During the time of inspection, there were 9 children present, between the ages of 11 months and 4 years, totaling 23 points. There was 1 caregiver present during this time.
