Comments:
An unannounced renewal inspection was conducted August 17, 2022 to determine compliance with licensing standards. The inspector arrived at 12:30 and concluded the inspection at 2:15pm. There were 12 children present, ranging in ages from 14 months to 12 years, with two caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of seven child records and four caregiver records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. If you have questions regarding this inspection, please contact the licensing inspector.

Based on review of four caregiver records, the provider did not obtain a current report of Tuberculosis Screening every two years from the date of the first screening for one caregiver.
Evidence:
The most recent documentation of a tuberculosis screening for caregiver #4 was dated 03/19/2018, exceeding the requirement to renew every two years. The provider stated caregiver #4 did not have a current tuberculosis screening.

Description:
Based on observation, the provider did not ensure all areas of the family day home, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
Three wooden pickets of the fence were broken creating jagged edges that could snag the skin of a child.
Plan of Correction:
Per the provider: I will ask my neighbor to help me with repairing the fence and replace those pickets.
