Comments:
An unannounced monitoring inspection was conducted on February 10, 2020 from approximately 3:30 pm 5:05 pm. Two staff were present with twenty school age children. Five children records were reviewed. Staff training and information to be maintained on site were reviewed for the two staff present.
Based on review, facility did not ensure 2 of 5 children records reviewed have documentation of required information.
Evidence: The records for child #2 (start date August 28, 2019) and child #4 (start date November 20, 2019) did not have father's employment phone number.
Plan of Correction:
Staff will get information from parents.

Description:
Based on review and interview, facility did not ensure there is documentation of completing at least two shelter-in-place drills per year and one lockdown drill per year.
Evidence: There was not documentation of completed shelter-in-place and lockdown drills completed in 2019 although staff report the drills were conducted.

All drills will be documented.
