An unannounced inspection was conducted today, August 19, 2019 in response to a complaint received by The Department on July 24, 2019 concerning the subsidy vender agreement. The inspection was initiated at 9:15am and concluded at 11:20am. Staff interviews were conducted, observations were made and documentation reviewed. Based on the information gathered during the investigation, the preponderance of evidence supports the allegation, therefore the complaint is valid.

Based on interview and observation, the center did not ensure compliance with the terms of the vender agreement.
Evidence:
1. The center owner/ vendor was in possession of four VA-ECC cards.
2. The center owner/ vendor pulled an envelope from a drawer containing four VA-ECC cards.
Plan of Correction:
Per the owner: I kept the subsidy swipe cards at the center because parents had asked me too because they would forget to bring them. I will return the cards to the parents today and inform them I am not allowed to hold the cards for them. One card belonged to a staff member and was returned while the inspector was on site.
