Comments:
A monitoring inspection was initiated on September 22, 2022 and concluded on September 26, 2022. There were 37 children present with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on observations, the center did not ensure all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe, and operable condition.
Evidence: During the inspection on September 22, 2022, there were two bathroom areas located inside classrooms #1 and #3 observed with areas of cracked paint and rusting throughout nearby the toilet. The metal parts were observed to be loose and rusted protruding with nails.

The center put in work orders to address areas.

Description:
Based on observation, the center did not ensure all hazardous substances are kept in a locked place using a safe locking method that prevents access by children.
Evidence: In Classroom#3, there were two bottles of dishwashing liquid located on a shelf unlocked with the labels "keep out of reach of children" and "warnings".
Plan of Correction:
The center staff immediately locked hazardous substance during the inspection.
