An unannounced monitoring inspection was conducted beginning at 10:20 a.m. and ending at 12:15 p.m. with the director. There were 37 children in care ranging in age from 2 to 5 years of age under the care and supervision of 8 staff. Children were observed during morning snack, free play and story time. Center documents and eight staff records were reviewed.

Based on review of documentation and interview with staff, it was determined that the center did not ensure a daily simple health screening and exclusion of sick children was conducted by a person trained to perform such screenings. Evidence: There was no documentation of Daily Health Observation training
Plan of Correction:
Daily Health Observation will be obtained as soon as possible by at least one staff member that is on duty at all times.
