Two staff were present with eleven preschool age children. Children were observed napping and eating a afternoon snack. Five child and five staff records were reviewed. Violations were found and are noted on the violation notice. Violations: Standard #: 8VAC20-780-260-B Description: Based on review, facility did not ensure there is annual approval from the health department. Evidence: The most recent health inspection report available for review was dated 9/13/2022 which is more than a year old. Staff confirmed a more recent inspection has not been done.
