Comments:
An unannounced monitoring inspection was conducted b3/29/2023 etween the hours of 12:15pm through 12:50pm. There were a total of 6 children present , ages 3 months through 2 years (18 points) with the Provider and 2 assistants. A total of 6 children?s files, 3 staff files, and 1household member?s files were reviewed. The children were engaged in free play activities, napping and infant feeding was observed. Positive interactions between caregivers and children were observed during this inspection. A complete inspection of the programming space, children and staff records, emergency drills, menu, attendance, and emergency procedures were completed today. No new staff have been hired since the last inspection. The provider advised there was no medication on site today. There was an adequate supply of age appropriate materials and toys. Areas of non-compliance are identified in the Violation Notice.

Based on record review, the provider did not obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.
Evidence: Child #3 (First Date of Attendance: 11/2/2022) did not have a physical examination on file at the time of inspection.
Plan of Correction:
We asked the parents of child to bring a copy of physical examination and they promised to bring it as soon as possible.

Description:
Based on record review, the provider did not ensure that children's records contained all required information:
Evidence: 1. Child #1 and Child #6's records did not contain the policy number of the children's medical insurance. 2. Child #3 and Child #6's records did not contain a second required emergency contact including a name, address, and phone number in case of emergency if the parent cannot be reached.
Plan of Correction:
We asked the parents of those children to provide the mentioned information and they did so.
