Comments:
An unannounced monitoring inspection was conducted today from 2:00pm-4:00pm. There were 9 children (1 child was the Provider's Daughter) for a total of 18 points with two caregivers. A sample size of 4 children's records, 3 caregiver/household member records, the home, and emergency materials were reviewed. The children were observed taking a nap, eating snack (crackers, bananas, and water) painting, and playing with toys. If you have any questions, please contact me at shahana.green@dss.virginia.gov .

Violations:
Standard #: 22VAC40-111-100-A
Description: Based on a review of children's records 1 out of 4 children's records did not have a physical examination under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance. Evidence: 1. Child #1 (start date: 2/20/2019) did not have documentation of a physical examination on file.

Plan of Correction: I will ask the parents for a physical exam for the child.

Standard #: 22VAC40-111-40-A
Description: Based on observation, the Provider did not ensure that the total number of children receiving care at any one time does not exceed the maximum licensed capacity of the home. Evidence: 1. There were a total of 9 children observed in the home. The Provider's licensed capacity is 7. 2. After an interview with the Provider, it was determined that 1 child in care was her daughter, which meant she was over her licensed capacity by 1 child.

Plan of Correction: I will make sure that I do not take more than 7 children.

Standard #: 22VAC40-111-200-B
Description: Based on a review of caregiver records, orientation training was not documented for all Assistant Caregivers. Evidence: 1. There was no documentation of orientation training for Caregiver #2 (start date: 3/13/2019).

Plan of Correction: I will complete orientation and place it in the file.

Standard #: 22VAC40-111-680-B
Description: Based on observation, the caregiver did not ensure that children's hands were washed with soap and warm running water before eating snack. Evidence: 1. 4 out of 9 children were observed eating afternoon snack (crackers and bananas) without washing their hands.

Plan of Correction: We will make sure the children wash their hands before and after eating snack.

Standard #: 22VAC40-191-60-C-2
Description: Based on a review of caregiver records, 1 out of 2 caregivers does not have documentation of a central registry finding within 30 days of employment. Evidence: 1. Caregiver #2 (start date: 3/13/2019) did not have documentation of a central registry finding on file.

Plan of Correction: I will follow-up on the one that I sent or I will send a new one.
