Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interview, and a virtual tour of the program. A monitoring inspection was initiated on 1/6/2022 and concluded on 1/07/2022. The Provider was contacted by telephone and a virtual inspection was conducted. There were 4 children (11 points) present, with 1 caregiver supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 2 child records, 1 caregiver record, 2 household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on a review of household member records, 1 out of 2 household members did not obtain a Child protective services central registry check within 30 days of becoming 14 years old.
Evidence: 1. Household Member #2 did not have results of a Child protective services central registry check on file during the inspection.

We will send the central registry check out right away.

Based on a review of household member records, each adult household member did not have a current Report of Tuberculosis Screening.
Evidence: 1. Household Member #1 did not have a current Report of Tuberculosis Screening form on file during the inspection.
Plan of Correction:
We will make sure the household member gets the TB screening right away.
