A Subsidy Health and Safety inspection was conducted on 11/06/19 to review supplemental health and safety requirements in response to a complaint received in the licensing office on 11/05/19. A licensing complaint inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.
Based on a record review and interview, the vendor failed to inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional. Evidence: On 11/05/19, a 22 month- old child #1 sustained an injury at the center to the right femur, requiring outside medical care. A serious injury online report or any other type of notification was never received by the department's representative
