Technical Assistance:
Technical assistance was provided regarding the requirements for tuberculosis tests and screenings. Tuberculosis tests and screenings must be signed by a physician, physician's designee, or health department official. In addition, consultation was made with the director regarding resilient surfacing.
Comments:
An unannounced monitoring inspection was conducted today from 12:20pm-2:35pm. There were 59 children directly supervised by 9 staff and all classrooms were in ratio. The physical plant, outdoor playground area, programming, menus, 5 staff records, 7 children's records, 5 medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. All staff files reviewed contained documentation of current CPR/First Aid certification and adequate staff on site are trained in Medication Administration Training and Daily Health Observation Training. Children were observed napping and infants were observed napping and playing with staff. There was an abundant supply of age appropriate materials for the children and the center was clean and organized. Areas of non-compliance are identified in the Violation Notice.

Based on record review, 1 of 7 children's records reviewed did not contain documentation of additional immunizations once every six months for children under the age of two years.
Evidence: Child #6 is a child under the age of two years and the most recent immunizations on file were dated 4/4/19.

Child recently had one year appointment. School had requested updated immunization record.

Description:
Based on review of 5 medications and medication authorzization forms, it was determined that two children had medications on site without signed parental authorization to administer the medications.
Evidence: Child #8 and Child #9 each had medications on site and did not have signed parental authorization granting the center permission to administer the children's medications.
Plan of Correction:
Medication forms on file were correct. Parent Authorization forms on file had expired. Updated parents authorizations are now on file.
