Comments:
An unannounced monitoring inspection was conducted 11/17/2022 from 11:05am to 11:35am to investigate a serious injury report received by the Northern Licensing Office. Observation of the physical plant was made, documentation was reviewed, and interviews were conducted. A violation was found regarding the physical plant and is identified in the Violation Notice.
Please provide a plan of correction for the noncompliance and include the date to be corrected. Please return a signed and dated copy to me by 5pm on 12/7/2022. If you have any questions regarding this inspection please contact me at morgan.bryson@doe.virginia.gov.
Based on observation of the physical plant, review of documentation, and interviews, the center did not ensure that areas and equipment in the center were maintained in a safe condition.
Evidence: In the Preschool Explorers classroom, a wooden bookshelf, approximately 3 feet tall by 3 feet wide, was not kept in a way that prevented tipping. On 11/4/2022 Child #1 (Age 2.5 years) "was playing with a friend and pulled the bookshelf down on top of Child #1 hitting the child's head causing a bump and cut on the child's eye" as stated in the accident report.
