Comments:
An unannounced renewal inspection was conducted on 08/07/2023. There were 6 children, ages 5 years ? 11 years, totaling 5 points and 1 caregiver present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment, and equipment, preventing the spread of disease, emergencies, and nutrition. A total of 3 children?s records, 1 caregiver?s record, 1 household member?s record and 3 pet records were reviewed. The children were observed during center time and lunch time. The inspector arrived for the inspection at 11:55am and departed at 1:29pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on the review of three children?s records, the provider failed to obtain documentation of a physical examination by or under the direction of a physician prior to a child?s attendance.
Evidence: 1. Child #1 first date of attendance was 06/06/2022 and on the day of the inspection the did not have documentation of a physical examination. 2. Child #2 first date of attendance was 05/15/2023 and on the day of the inspection the did not have documentation of a physical examination.

Evidence: The most recent rabies vaccination for Pet #3 expired on 7/14/2023 and on the day of the inspection a current rabies vaccination was not documented.

Provider will obtain current rabies vaccination for pet.
