Comments:
An unannounced monitoring inspection was completed today. The physical plant, 3 caregiver/household member files, 5 children's files, caregiver training, child nutrition, children's activities, emergency procedures, drills and supplies were inspected between 12:15-2:00pm. There were 10 children, totaling 32 points, under the supervision of 3 caregivers. Today, the children were fed bottles (infants), listened to music, cleaned up for and rested during naptime. The interactions between the children and caregivers were seen to be positive and constructive in nature. Today, the children were served noodles with cheese (chicken for non-vegetarians), mixed vegetables, strawberries and milk. The Provider is certified in MAT (medication administration training). Based on file review, the Provider did not have documentation of having completed a monthly evacuation drill with all caregivers and children in care during all shifts that children are in care. Evidence 1. The most recent evacuation drill that had been documented, was completed on 7/3/19.

Based on file review, 2 out of 5 children's records do not have additional immunizations every six months up to the age of 2 years. Evidence 1. Children #2 and #3 are 3 years old and do not have their 2 year old immunization records on file.
