Comments:
An unannounced monitoring inspection was conducted on October 30, 2023 from approximately 3:15 pm to 4:45 pm. Four staff were present with two school age children at the onset of inspection. Additional children and staff arrived during inspection. Five child and four staff records were reviewed. Violations were found and are noted on the violation notice.
Based on review, facility did not ensure required background check is completed within 30 days of employment.
Evidence: The record for staff #1 (start date 5/23/2023) did not have a completed central registry search.
