Comments:
An unannounced complaint investigation was conducted today in regards to a complaint received by the Fairfax Licensing Office on 9/19/2023 in regards to Emergency Procedures and Medication Administration. Interviews were conducted and caregiver training was reviewed.
A preponderance of the evidence supports the allegation and it is deemed ?valid?. Violations related to the complaint can be located on the Violation Notice.

Based on staff statement, topical skin ointments were applied without written authorization. Evidence: On 5/26/2023, petroleum jelly was applied to a laceration on Child 1?s forehead. Child 1 did not have a written authorization on file permitting the use of petroleum jelly.

On 5/26/2023, Child 1 hit their head on a rocking chair resulting in a laceration on the child?s eye brow that was deep enough to require stitches. 2.
