Comments:
An unannounced renewal inspection was conducted on 6/26/23 from 9:40 a.m. to 10:40 a.m. There were three children, ages 23 months to 5 years for a total of nine points, with the Provider. The programming area, attendance, outdoor playground area, menus, Provider, Assistant and Household member records, three children?s records, emergency drills, injury reports, emergency supplies, and policies were reviewed. No medication was on site. Children were observed eating breakfast and free play. There was an adequate number of staff present with CPR and First Aid training. Areas of non-compliance are identified in the Violation Notice.
Please complete the columns for "Plan of Correction" and "Date to be Corrected" for each violation cited on the Violation Notice, and then return a signed and dated copy to the licensing office by 5pm on July 10, 2023. Based on record review and interview, the provider did not obtain a sworn disclosure statement for each household member living in the residence above the age of 18 years.
Evidence: The record for household member #1, with a move in date of 6/25/23, did not have documentation of a sworn disclosure statement at the time of the inspection.
