Comments:
A Renewal Inspection was initiated and concluded on July 26, 2022. There were 2 children present under the supervision and care of the Provider. The Inspector review 2 children records and 1 household/staff record, along with any requested program records submitted by the Provider to determine if required documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or laws and violations were documented on the violation notice issued to the Provider.

Based on review of records, the Provider failed to maintain an individual record for each child enrolled in care which includes all required information & documentation.
EVIDENCE: Licensing Inspector reviewed 2 children records.
Record for Child #1 did not include up-to-date immunization record; medical release form; liability Insurance disclosure; proof of birth/identity.
Record for Child #2 did not include proof of birth/identity.

Provider will request information from the child's parents. Child's record will be updated with current documents.

Based on inspection, the Provider failed to meet the criteria on the health and safety checklist at the time of the renewal inspection.
EVIDENCE: Monthly emergency evacuation drills have not been completed.
