Comments:
Licensing Inspector conducted an unannounced Subsidy Inspection on May 23, 2024. There was one 15 month old child present under the direct care of the Provider. The Licensing Inspector reviewed 1 child record and 1 household/staff record, along with any requested program records submitted by the facility to determine if required documentation was complete.
Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the program.
Based on record review, the Vendor failed to update required background checks 5 years from the date of the most recent background check.
EVIDENCE: Staff #1 Fingerprint Background Check was not updated 5 years from the date of the most recent Background Check. Most recent Fingerprint BC was completed 6/08/2018.
