Comments:
A renewal inspection was conducted on February 27, 2024, at 9:20am until approximately 10:10am. There were two children present and the provider. The Inspector reviewed three children?s records and the providers, along with any requested program records submitted by the facility to determine if required documentation was complete. Children were observed during breakfast, and free play. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed with the provider during the exit interview.
Based on record review and interview, it was determined that the provider did not ensure that all applicants shall undergo a background check in accordance with subsection B every five years thereafter. Evidence: 1. Staff #1, provider, has an expired Virginia Criminal Background check dated May 14, 2018. The expiration date for this is May 14, 2023. 2. Staff #1 confirmed that their Virginia Criminal Background Check was expired as of May 14, 2023.

The provider responded with the following: The provider will obtain their Virginia Criminal Background Checks and send the results to licensing once received.

Description:
Based on record review and interview, it was determined that the provider did not ensure that they maintain an individual record for each child enrolled in care. This record shall include up-to-date immunization records. Evidence: 1. Child #1 (date of enrollment 02/24/2023) did not have an up-to-date immunization record in their file. 2. Staff #1 confirmed that Child #1 did not have an up-to-date immunization record in their file.

The provider responded with the following: The provider will obtain the child?s immunization record and place it in the child?s record.
