22VAC40-180 PROVIDER REGISTRATION AND GENERAL PROCEDURES 22VAC40-180 HEALTH AND SAFETY CRITERIA AND CHECKLIST.
Comments:
Licensing Inspector arrived at 11:30 AM and departed at 12:30 PM to conduct a voluntary registration renewal inspection. At the time of the inspection, there were two children in care. Children were observed sleeping and watching television. The sample size was four children's records. An exit interview and review of violations was held at the conclusion of the inspection.
Based on review of four children's records, it was determined that the provider did not ensure that all required information was present in each child's record.
Evidence: 1. The records for child #1 and child #3 did not contain addresses for persons designated by the parent to be called in case of an emergency when the parent can not be reached. 2. There was no record available for child #5 who was in care at the time of the inspection. 3. The records for child #1 and child #3 did not contain results of a health examination.
Plan of Correction:
Provider will work with parents to ensure that all children have complete records.
