The following was discussed with the program director:
Food group components for afternoon snack

Comments:
A renewal inspection was initiated and concluded on 07/02/2024. The inspector arrived at 11:25 AM and departed at 2:15 PM. There were 106 children present, ranging in ages from 6 to 12 years, with 12 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 6 child records and 6 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program and discussed with the program director and administrator during the exit interview

Violations:
Standard #: 8VAC20-780-60-A
Description: Based upon review of children's records, the facility has not ensured that the records include the business telephone numbers for the parents/custodians.
Evidence:
The records provided for child 1, 2, 3, 4, 5 and 6 did not include business telephone numbers for the parents as the program used to enroll the children does not ask for that information.

Plan of Correction: The facility responded with the following:
A Google form has been created to send to all parents to capture work/business numbers.

Standard #: 8VAC20-780-240-A
Description: Based upon review of staff records, the facility has not ensured that the Virginia department of Education (VDOE)-sponsored orientation course was not competed within90 calendar days of employment.
Evidence:
According to the record provided, staff 1 was employed on 6/21/2019. Staff 1 has not completed the VDOE sponsored orientation course or the required three hour annual updates trainings.

Plan of Correction: The facility responded with the following:
All new staff will be asked to complete the required 10 hour VDOE training this year; and returning staff advised to complete the three hour training.

Next year, new candidates will be asked to complete the training as a pre-requisite for hire. And returning staff will be instructed to complete the 3 hour by May 1 2025.

Standard #: 8VAC20-780-260-B
Description: Based upon review of records, the facility has not ensured that they have annual approval from the health department to met requirements for the water supply and sewage disposal system.
Evidence:
The facility had annual approval for food service but did not have an annual inspection of the water supply and sewage disposal system.

Plan of Correction: The facility responded with the following:
The Exec. Director met with the Head of School and Finance for Norfolk Collegiate on 7/10/2024 and they agreed to reach out immediately to the health Dept to request the inspection. They will share the inspection results as soon as it is complete and the report will be forwarded to VDOE at that time.

Standard #: 8VAC20-780-510-E
Description: Based upon review of medication, the facility has not ensured that their procedures for the administering of medication requiring written parental permission for medications are followed.
Evidence:
There is diphenhydramine and epinephrine stored for emergency administration to child 7. There is no written parental permission for the administration of these medications.

Plan of Correction: The facility responded with the following:
The parent has returned the signed form. We also sent the medication authorization form to all parents of students with medications.
