Comments:
An unannounced monitoring inspection was conducted on November 16th, 2022. There were 12 children present (33 points) with 2 caregivers. The home, attendance logs, evacuation logs, cribs, bottles, diaper creams, children's records, caregiver records, household member records, first aid kits, and menu's were reviewed. The children were observed playing with floor toys, working on their alphabets and two children were sleeping. Area's of non-compliance were cited on violation notice. Based on review of children's records, the provider did not annually review with the parent the emergency contact information (as required in 8VAC20-800-60-B).
Evidence: Child #1's record did not contain documentation of an annual review. The last review date for emergency information was 06/14/2022.
Plan of Correction:
I will complete the annual review with the parents and place document in child's file.

Description:
Based observation and attendance log, the provider did not ensure that caregivers only care for children totaling up to 16 points by using the point system based on the children's age.
Evidence: There were 12 children present, totaling 33 points with 2 caregiver's present.

I have hired another assistant; however, her documents are not completed.
