Comments:
A renewal inspection was conducted on 1/20/2023. There were a total of 12 children (30 points) observed with 3 caregivers present. The children were engaged in the following: toy play, games, snack, bathroom breaks, hand washing, and other organized activities. The inspector reviewed compliance in the areas of administration, personnel and household members, physical environment, care of children, handwashing, diapering, medication, emergencies and nutrition. A total of 5 child records and 4 caregiver/household member records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Please contact me if you have any questions at Ariel.Hayes@doe.virginia.gov or 804-629-7124.

Violations:
Standard #: 8VAC20-800-320-B
Description: Based on observation of the physical plant, child-resistant protective covers larger than 1-1/4 inches in diameter were not installed on all unused electrical outlets accessible to children under five years of age.
Evidence:
Several unused electrical outlets did not have child-resistant protective covers in the room closest to the exit door that leads to the drive way.

Plan of Correction: I have put outlet covers in all of the open electrical outlets.

Standard #: 8VAC20-800-460-D
Description: Based on a review of documentation and provider interview, the provider did not maintain documentation of the current rabies vaccination.
Evidence:
1. There was no documentation of current rabies vaccination for 2 of 2 dogs.
2. The provider confirmed that she did not have documentation of the rabies vaccination.

Plan of Correction: I have an appointment with the vet scheduled on 1/25/2022.

Standard #: 8VAC20-800-480-D
Description: Based on observation, equipment used by children was not used in accordance with the manufacturer's instructions.
Evidence:
One child was observed sitting at the table in the toddler seat not strapped in with the provided safety buckle.

Plan of Correction: I will talk to my assistants about buckling in the children when they are at the table.
