An unannounced "Code of Virginia Section 63.2-1716" compliance monitoring inspection was conducted from 8:00 AM to 11:50 AM. Twenty-four children, ages 4 months through 5 years old, were under the care and supervision of 5 staff. Center documents and seven staff records were reviewed.

Based on review of documentation, the facility did not to ensure that satisfactory central registry background checks were obtained with 30 days of employment. Evidence: Staff A, start date 4/16, Staff B, start date 8/10, Staff C, start date 9/18, Staff D, start 8/16, Staff E, start 8/10, Staff F, start 9/17, and Staff G, start date 6/18 did not have documentation of a central registry background check results in the record.
Plan of Correction:
The center was provided a copy of the request for central registry check. The central registry background checks will be completed for all staff and placed in the record.

Description:
Based on review of documentation and interview with staff, the facility did not provide written disclosure of required information to the guardians of the children in the center and the general public. Evidence: There was no written statement provided regarding physical facilities, enrollment capacities, health requirements for staff, and public liability insurance.
Plan of Correction:
The center director was provided a copy of the Public Disclosure Statement form. The form was completed during the inspection and posted fin the lobby area of the center.
