Comments:
An unannounced monitoring inspection was conducted fro 9:52 am to 10:25 am. There were 9 children (25 points) in care of three caregivers. The children were observed playing with toys, getting bottle fed, coloring and doing a number learning activity. There were no medications on site. 5 children's, 1 caregiver and 1 household member record was reviewed. Areas of non compliance are listed in the violation notice. Based on record review, the family day home did not obtain additional immunization records for children in care. Evidence: Child # 1's first date of enrollment was April 2015. The date of the most recent immunization records obtained for Child # 1 were dated October 2015. Additional immunization records were not obtained for Child # 1 since that date.
