Comments:
A joint investigation was conducted with the local agency and law enforcement on May 20, 2024, from 10:25 a.m. to 11:06 a.m. to investigate a self-reported incident regarding forbidden action: rough handling. Staff interviews were conducted. Facility documentation was reviewed. Additional interviews were conducted on May 22, 2024. Based upon the information gathered during the investigation, violations were cited and can be found on the violation notice.

Based on interviews, it was determined that the center failed to ensure that roughly handling children was forbidden. Evidence: It was determined that on May 10, 2024, Staff #1 roughly handled Child #1, age 3 years, by roughly holding him by the upper arms leaving bruising.
