The licensing inspector conducted an unannounced Subsidy Health and Safety inspection on October 19, 2023 from approximately 9:20 AM to 11:45 AM. There were 21 children in attendance and a total of three staff supervising. A total of five children and five staff records were reviewed. A code compliance inspection was conducted on the same date as the SHSI supplemental inspection.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of five staff records and interview, the vendor did not ensure one staff member obtained documentation of subsequent background checks at least every five years. Evidence: 1. The record for Staff #5 (Date of Employment 07/27/2015) contained satisfactory results of the fingerprint-based national criminal background check dated 04/14/2018, a sworn statement dated 03/28/2018, and the central registry finding could not be located during the inspection. 2. Staff #3 confirmed the information during the inspection.

Description:
Based on observation, the vendor did not ensure all hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevented access by children. Evidence: Cleaning chemicals were in the cabinet underneath the sink in the bathroom. The cabinet did not contain a locking method. Children were observed using the bathroom during the inspection.
