Technical Assistance:
Consultation provided on outside time for infants- weather permitting;
consistently documenting the time of day parents were notified of injuries to their child; and varying the time of day you do your monthly escape drills

Comments:
An unannounced renewal study was conducted with the director, who was present with 19 staff who were providing supervision to 86 children , from 4 months to 6 years of age. The infants were napping or crawling around on the floor, while the the toddlers/ two's were eating their morning snack ( milk and graham crackers) and the older children were practicing their singing for the Winter Concert. Six medications for four children were reviewed as well as 12 injuries/accident reports. Six staff and seven children's files were reviewed today as well.
Within 14 days please contact Licensing with the date that the background checks were initiated/sent off for the Agent/Board Member. You will need to contact Licensing within 30 days to verify that all of the background checks have been obtained.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation,it was determined that the outside equipment and space was not maintained in a safe and operable condition.
Evidence:
The resilient surfacing ( poured-in-place) surrounding the green, tan and burgundy climbing structure on the older playground was in disrepair as there was an approximate 8 inch divot in the poured in place.

The ground covering on the older playground with the picnic table was exposed and presented a tripping hazard.

Plan of Correction: We will place the structure off limits until repaired and we will cover or cut the ground covering so that it no longer presents a tripping hazard.

Standard #: 63.2(17)-1721.1-A
Description: Based on a review of record keeping and an interview with the director, it was determined that an Agent did not have repeat background checks every five years.
Evidence:
Agent A's Sworn Disclosure statement was dated 6/3/13 and the Central Registry Finding was dated 1/22/13, neither of these background checks have been repeated in the last five years as required.

Plan of Correction: We will get the new background checks.

Standard #: 63.2(17)-1721.1-B-2
Description: Based on a review of record keeping and an interview with the director, it was determined that an Agent and a Board Member did not have fingerprint background checks, which were required by 9/30 /18
Evidence:
Neither the Board Member nor Agent A had fingerprint background checks on file.

Plan of Correction: They will get these background checks and we will notify Licensing when they get them done and when the results have been obtained.
