Comments:
An unannounced renewal inspection was conducted today with the director and administrative staff from 9:10 am to 11:35 am. There were 135 children present with 16 staff providing supervision. The ages of the children ranged from 4 months to 6 years. During the walkthrough the Licensing Inspector was able to observe circle time, several classrooms outside on the playgrounds, morning snack time to include handwashing, staff's knowledge of their children with allergies/sensitivities/diet restrictions. A review of recordkeeping included an inspection of 7 children and staff files, medications for 10 children and a sample size of 8 injuries/accident reports. The most recent fire inspection was dated 10/11/23 and the health inspection was dated 9/11/23. The area of non compliance is outlined on the violation notice.

Violations:
Standard #: 8VAC20-780-330-B
Description: Based on observation, it was determined that there was insufficient resilient surfacing in front of the green slide on the older playground.
Evidence:
There were 4 divots in the resilient surface ( poured-in-place) 1 to 3 feet in front of the slide. The divots made the poured-in-place surface out of compliance with the minimum safety standards for resilient surfacing.

Plan of Correction: We will place the slide off limits to the children by using caution cones and signage and by training staff to ensure that they help block access by the children.
