An unannounced inspection was conducted today, 1/30/2020. Violations from the previous inspection was reviewed for compliance. There were 27 children in care, ranging in age from two months to five years with eight staff present. Should you have nay questions please do not hesitate to contact your inspector. Inspection began: 10:15 am ended 12:10 pm.
Based on interviews the provider failed to have all new staff submit to a sworn statement or affirmation before employment and central registry checks within 30 days of employment. Evidence: 1. Staff # 1, 2, 3 do not have a sworn disclosure statement or affirmation on file before employment. All three staff were working the day of inspection 1/302020. 2. Staff # 1 & 3 do not have a central registry check on file within 30 days of employment. (Staff #1 hire date 9/17/19, staff #3 hire date summer 2019)
