Comments:
A monitoring Code compliance inspection was initiated on 8/12/2022 and concluded on 8/12/2022. There were 11 children present with two staff supervising, a third staff came in at approximately 11:15 am. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and four staff records were reviewed. The inspection started at 10:00 am and concluded at 11:31 am. Please complete the ?plan of correction? and date to be corrected? for each violation cited on the violation notice and return it to me within five (5) business days from today. You will need to specify how the deficient practice will be or has been corrected. Just writing the work ?corrected? is not acceptable. Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring gain; and 3) person(s) responsible for making the corrections.
Based on observation the center failed to ensure that the ratios of one staff member to four children from ages zero to 16 months was followed. Evidence: 1. Upon arriving to the center two LI's observed six children in the infant area with one staff. The youngest was eight months and the oldest was eight years. 2. Three of the children were participants in the school New Destiny. Staff #3 was the teacher for the school age children and caring for the three infants.

Based on record review the center failed to ensure that all staff submit to fingerprint-based criminal history checks prior to the first day of employment. Evidence: 1. There are no finger-print background checks in the records of staff #2 (date of hire 6/2022; staff #3 (date of hire 11/11/2018 and staff #4 (date of hire 6/1/2019)
