Comments:
An unannounced monitoring inspection was conducted on 10/11/2022. There were 29 children, ages 6 weeks-4 years and 9 staff present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, care of children, preventing the spread of disease, emergencies and nutrition. Medications for children were observed during the inspection. A total of 5 children?s records, 5 staff records and 5 medications were reviewed. The children were observed during circle time and active time on the playground. The inspector arrived for the inspection at 9:57am and departed the center at 11:13am.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
Based on review of five children?s records, the center failed to ensure that all physical records shall be signed by a physician or designee.
Evidence: Child #2 started on 5/27/2022 and on the day of the inspection did not have the required signed physical examination record.
Plan of Correction:
Provider will obtain a signed physical record.
