Comments:
An announced monitoring inspection was conducted today from 10:00am-11:40am. Staff files were reviewed at the Loudoun - YMCA's main office in Leesburg. Two staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the program did not keep documentation of training in a manner that allows for identification and that includes, training delivery methods and the number of training hours.
Evidence: Staff #1's record contains 6 training certificates that do not indicate the number of training hours or the training delivery method.
Plan of Correction:
We have updated our documentation method to track training received by employee throughout the duration of their career. The document will contain the name or type of training along with hours received and delivery method. We have also reached out to our corporate office to gather all documents pertaining to employees who have been with the facility over the past few years and will maintain those records at the localized branch.
